Description
The Primary Care Coalition (PCC) seeks a Temporary (4 month assignment) Hospital Outreach and Health Education Specialist to perform street-outreach activities linking individuals experiencing homelessness to comprehensive housing and social services supports. The PCC partners with the Montgomery County Healthcare for the Homeless Program to provide case management and patient education and support services for program participants. Health Care for the Homeless supports adults and families experiencing homelessness in Montgomery County. Program clients include formerly homeless individuals some of whom have been frequent users of the emergency medical and corrections systems with permanent supportive housing and case management services. Program services include intensive on-site case management; life skills training; assistance in acquiring a primary health care provider and accessing mental health and drug treatment services; assistance in obtaining benefits and entitlements; linkages to appropriate services available in the community including food, clothing, workforce development opportunities, psychiatric rehabilitation day program.

This role will work closely with the Healthcare for the Homeless Program Manager to foster relationships with hospitals and community based organizations to create referral pathways to housing programs for people experiencing homelessness in Montgomery County. Must have reliable transportation and full Covid19+ vaccinated (must provide documentation).

Responsibilities Include:

• Under direction of program manager prepare hospital discharge planning documents, COVID-19 motel assignments, and state reports as required.
• Provide person centered health education, case management and care coordination services to assist clients. Serve as an advocate and represent clients’ interests by liaising with landlords, hospitals, medical providers, family members, and other connection points relevant to managing client cases and addressing client needs.
• Provide intensive case management services to clients presenting in the emergency room, to prevent overuse of the emergency rooms and to reduce readmission to hospitals. Serve as primary contact and liaison to hospitals and other medical facilities requesting referrals for nursing assessments.
• Maintain community relationships to create referral pathways to housing programs for people experiencing homelessness in Montgomery County and referred to as a “high utilizer” of County health care resources. 
• Attend workgroups, hospital meetings, and community collaboration meetings. 
• Work as an active member of the diversion team regarding cases referred through County hospitals and other medical facilities, as requested. 
• If requested, provide health education presentations, both in person and virtually, at all single and family shelters and transitional shelters. 

MINIMUM QUALIFICATIONS: 

• Bachelor’s degree from an accredited college in human services, health field, or related field Masters in Public Health preferred. 
• Three years work experience in social services.
• Previous street outreach experience or substantial experience working with individuals and/or families experiencing homelessness. 
• Demonstrated knowledge of housing first approaches, housing focused outreach efforts, and motivational interventions preferred. 
• Strong oral and written communication skills.
• Basic knowledge of Microsoft Word and Excel, and a database system.
