Would you like to keep video gaming within a healthy and balanced life for yourself or someone you care about?

The purpose of the following survey is to help us better understand video gaming patterns. Your answers will help us guide those with high-risk gaming choices. 

We don’t see gaming as “good” or “bad,” and want to understand any needs you might have for keeping gaming safe and healthy. 
We expect this survey to take about 5 minutes to complete. Your responses are anonymous.

Following the survey, you will have a choice to follow a link to be entered into a drawing for a $20 Amazon gift card.

Your participation in this study is completely voluntary.
