Caregiver Insights with Newly or Recently Diagnosed Children with Dravet Syndrome |
Survey Consent
Dravet Syndrome Foundation (DSF), in partnership with a pharmaceutical company named UCB Biosciences, is looking to better understand the experiences and unmet needs of caregivers of children with Dravet syndrome who are 4 years old or younger. We would greatly appreciate if you could complete this 10-15 minute survey to help us better serve this community.
De-identified data from this survey will be shared with UCB. Insights from the compiled survey data will be used to develop infographics and other communications to share with the community and the broader public.
The only identifying information that will be shared outside of DSF will be from caregivers that 'opt-in' to share their contact information with UCB at the end of the survey. This contact information will be shared separately and will not be tied to other survey responses.
