DSF Strategic Plan Feedback Survey 

A strategic plan is a tool that provides guidance in fulfilling an organization's mission and goals. DSF is currently developing the 2023-2028 DSF Strategic Plan, and we need your help to gather information on what is most important to you and your family.

Your responses to the following questions will only be presented in summary form, grouped anonymously with other survey respondents. This information will help us assess how we are supporting the diverse communities impacted by Dravet syndrome. This survey is estimated to take 20-25 minutes and must be completed in a single sitting.
