Description
The Director, Clinical and Practice Transformation, Montgomery Cares will recommend and oversee clinical quality improvement initiatives for MCares population. They will lead practice transformation initiatives for Montgomery Cares primary care providers, recommend network improvement for MCares primary care, BH, specialty care, dental networks, research, design and recommend program improvements for MCares program overall to achieve optimal health of the population, excellence in patient experience, a culturally competent program that addresses disparities and works to achieve equity in service and outcomes, all at a value for DHHS and other stakeholders, preserve and strengthen partnerships and relationships with DHHS PHS and MCares clinics.

Primary External Relationships:  
• Montgomery Cares primary care clinics: Medical Directors, Executive Directors, Clinic Managers
• DHHS: Chief of Policy and Strategy

Essential Activities

1. Conduct Design of Program Elements:  Work closely with local health department and clinic and health center leadership to support program improvements, including the transition from “Volume to Value” and care model transformation in a safety-net primary care setting. Areas of transformation include but are not limited to:
• Network adequacy standards for primary, specialty, behavioral health, oral health, and medicine access
• Provider network participation standards
• Value based care 
• Reimbursement (FFS and value-based payment models)
• Clinical quality measures, benchmarks, and targets

2. Facilitate Program Design, Policy and Procedure Decisions: Convene meetings, disseminate other communications to facilitate collective input to program design areas.  Develop and foster collaborative relationships to ensure sufficient input from those with relevant experience, and sustainability of the public-private partnership. Facilitate equitable and open communication among stakeholders. Maintain and communicate project plan with timelines and responsibilities for major program design and policy element decisions.

3. Provide Technical Assistance:  Directly provide, and oversee staff and consultants that provide, change management and practice transformation to support safety-net clinics in achieving targets set for clinical quality, value-based care, and practice effectiveness measures. 

• Support primary care clinics in practice transformation to meet changing program design goals. Enhance practice management capacity among the safety-net clinics including for implementation of practice transformation models (e.g., PCMH, CMS practice transformation assessment tools). Activities include provision of education, materials, external technical assistance resources.
• With clinic leaders and DHHS, develop measures of practice effectiveness and patient experience, aligning measures and targets with relevant comparable benchmarks where possible (e.g., Maryland Medicaid star-rating, HCAPS). Maintain communication with clinic partners to understand requirements to facilitate care delivery and business operations.  Organize or provide technical assistance and process improvement as needed, within budget.
• In collaboration with Medical Directors and DHHS, support improvement in clinical quality at safety-net clinics. Facilitate the development of a set of clinical quality measures and targets, aligning with comparable national or state benchmarks where possible (e.g., HEDIS, Maryland Medicaid).  With the data analyst, develop the technical specifications for the measures of clinical quality, With the data analyst and clinics’ staff, ensure data is reported accurately and timely to PCC for analysis of clinical quality performance to targets. Oversee the quarterly and annual data reporting of clinical quality measures.  Facilitate quality and process improvement activities within the clinics as needed to bring performance to targets.

4. Promote use of data for evaluation and measurement of program effectiveness: Liaise with internal and external stakeholders to provide guidance on data strategy to facilitate effective transition to Value Based Care model of service delivery.  Promote the role of accurate data and effective technology use in quality and workflow improvement, and practice effectiveness.  Provide or oversee technical assistance to the primary care clinics to effectively leverage capabilities of the electronic health record for improvement in patient care, clinical quality and workflow, and to build clinic management capability for overseeing accurate data collection, data reporting, data interpretation and subsequent improvement actions. 

5. Oversee consultants conducting clinical quality assurance and patient experience audits.

6. Supervise staff members who conduct electronic health record help desk functions.  Ensure team has efficient training plans, and meets the training and customer service needs of client clinics.

7. Identify and direct grant-funded clinical quality improvement projects, including contributing to written grant proposals, and assuring the development and tracking of work plans, management of grant funds, and submission of timely updates and project evaluations to funders and other stakeholders. Identify and nurture relationships with potential new partner organizations.   Actively participate with PCC Board of Directors and program partners to advocate for resource needs and policy priorities related to system transformation.

8. Manage a small team dedicated to transforming clinical and practice excellence in the local safety-net health system. Hire, supervise, and develop department staff for an effective workflow to accomplish the department’s goals and objectives within budget. Ensure PCC policies and procedures are adopted and serve as a mentor and coach to direct reports willingly sharing knowledge and expertise and involving appropriate staff in planning, decision-making and problem solving. Clearly communicate performance expectations and provide direct reports with opportunities to develop skills and progress in their careers.

Skills and Abilities:

• Familiarity with data collection/data analysis, and relational data base functionality.
• Demonstrated experience in process improvement and change management with ability to work as a consultant, facilitator, and colleague with internal and external stakeholders.
• Ability to lead, facilitate and work with diverse groups; communicate effectively and persuasively, both orally and in writing, with a broad range of stakeholders and in charged environments of strongly held viewpoints.
• Ability to organize, develop, implement, monitor, and evaluate professional work plan goals and performance objectives.
• Demonstrated ability to conceptualize, direct and implement planning and quality improvement strategies.
• Demonstrated ability to develop collaborative projects and work with a variety of public and/or private providers, policymakers, and researchers.
• Self-motivated individual with the ability to work independently and with minimal supervision.
• Understanding of Spanish language or various Latin American cultures a plus. 

Education and Experience
 
• 5 to 10 years of increasingly responsible experience in safety-net health care delivery or health care administration in an ambulatory care setting. Clinical licensure a plus.
• Experience with clinical operations and practice transformation
• Experience addressing health disparities thorough an equity lens and knowledge of cultural competency best practices.
• Proficient in Microsoft Office suite of products (Outlook, Excel, Word, PowerPoint, SharePoint, Teams)
• Formal training in a Quality Improvement/Quality Management/Process Improvement methodology preferred
• Experience using an Electronic Health Record or involvement in workflow or quality improvements using data from an EHR. 

Telework

This position is approved for telework when activities do not require a physical presence.  The job duties require in-person meetings with team members, a variety of groups, State and clinic representatives, and other community partners.  It is an expectation to regularly attend in-person PCC meetings.
